Instructions for Completing Form W-9

1. Open the file using Adobe Acrobat. If you do not have the full version of Adobe Acrobat, you must print and enter the information by hand.
   
   
2. Enter Name as “Austin Intellectual Property Law Association”
   
   
3. Check “other” box for type of entity, and fill in “Nonprofit Organization 501(c)” on line next to “other” box
   
   
4. Fill in address (use address of current Vice President).
   
   
5. Fill in Employer Identification Number: 74-2422311
   
   
6. Print form and mail to our Treasurer for certification and signature. Please include a self-addressed, stamped envelope. After all, we are a nonprofit organization!
